Cardiac Consultation
A Cardiac Consultation occurs when your referring physician requests the expertise of a cardiologist, a medical professional specializing in the diagnosis, treatment, management, and prevention of heart disease. This consultation is intended to address specific concerns or manage complex cardiac conditions.
Request for Consultation
A Cardiac Consultation must be requested by a licensed medical doctor or nurse practitioner. If your referring provider includes diagnostic tests as part of the referral, it is essential that these tests are completed before the consultation. This ensures the cardiologist has all necessary results to make an informed assessment and provide effective care during your visit.
Referrals are triaged based on the cardiac status and indications provided by your referring physician. Patients with urgent concerns or specific symptoms may be prioritized. Once we receive the referral, our office will contact you to schedule an appointment and provide the date and time.
Follow-Up Appointments
Following the Cardiac Consultation and review of test results, the cardiologist may request additional diagnostic tests using other modalities. Completing these tests is critical to finalize the diagnosis, determine prognosis, and create an effective treatment or prevention plan. The consulting cardiologist will review all results and communicate next steps with our administrative team to ensure timely care.
Cardiac Re-Assessment
For patients with established Coronary Artery Disease (CAD), valvular disease, arrhythmias, or cardiac risk factors such as hypertension, diabetes, dyslipidemia, or a family history of heart disease, periodic cardiac re-assessments may be recommended. These evaluations allow the cardiologist to monitor progress, adjust treatment plans as needed, and manage ongoing care effectively.

Please arrive 10 minutes before your scheduled test to register at the reception desk.
Bring the following items:
Your valid OHIP (Ontario Health Insurance Plan) card (to each scheduled appointment).
Bring all your prescription medications to the consultation
A requisition form, if provided to you directly by your physician or nurse practitioner.
(Note: If your physician or nurse practitioner did not fax the referral to our office, you must bring the requisition form signed by your physician to your appointment.)
