Cardiac Consultation

A cardiac consultation is a specialized evaluation conducted by a cardiologist to assess, diagnose, and manage cardiovascular conditions. The consultation involves a comprehensive review of the patient’s medical history, symptoms, risk factors, and diagnostic test results (e.g., ECG, echocardiograms, stress tests). The cardiologist’s goal is to establish an accurate diagnosis, determine prognosis, and develop an individualized care plan that may include further testing, therapeutic interventions, or lifestyle modifications to optimize cardiovascular health.

Who Can Request a Cardiac Consultation?

Cardiac consultations can be requested by:

  • Referring physicians (licensed medical doctors)

  • Nurse practitioners

To support efficient triaging and informed decision-making, the referral should include:

  • Reason for the referral (e.g., evaluation of symptoms, management of a specific condition)

  • Relevant prior diagnostic procedures (e.g., imaging studies, stress tests)

  • Supporting documentation (e.g., ECG results, blood work conducted within the past four weeks)

Components of a Cardiac Consultation

  1. The Request:

    A formal referral from a qualified healthcare provider is required (MD, Nurse practitioner).

    Detailed patient information, including recent diagnostic findings, assists the cardiologist in making informed decisions.

  2. The Medical Examination:

    A thorough assessment comprising patient history, physical examination, and clinical decision-making is conducted to establish the level of care needed.

  3. The Report:

    A comprehensive written report is prepared, summarizing findings, diagnostic conclusions, treatment recommendations, and any therapeutic interventions initiated.

    This report is shared with the referring physician to ensure continuity of care.

Referral Triage

Referrals are triaged based on the cardiac status and clinical indications provided by the referring physician. Patients with urgent concerns or symptoms are prioritized. Once the referral is received:

  • Our office contacts the patient to schedule an appointment.

  • Referring physicians should ensure the patient’s contact information is accurate and up to date, especially for urgent cases.

Follow-Up Appointments

Following the consultation and review of diagnostic results, additional tests may be recommended to refine the diagnosis and treatment plan. Completing these tests promptly is crucial to ensuring optimal care. A follow-up consultation letter summarizing the cardiac status, test results, and updated recommendations will be provided to the referring physician.

Cardiac Re-Assessment

For patients with established cardiovascular conditions, including:

  • Coronary Artery Disease (CAD)

  • Valvular disease

  • Arrhythmias

  • Risk factors such as hypertension, diabetes, dyslipidemia, or a family history of heart disease

Periodic cardiac re-assessments are essential for ongoing management. The cardiologist will outline recommendations for future evaluations upon discharging the patient. These recommendations may be updated based on new clinical indications or evolving patient needs.
