FREQUENTLY ASKED QUESTIONS
How Do I Book an Appointment at Advanced Cardio Diagnostics (ACD MED)?
You need a referral from a medical doctor (referring physician) or a nurse practitioner to book an appointment at ACD MED. Your doctor can print a referral form directly from this website or call any of our locations to request one. Alternatively, you can download the referral form by clicking here, print it, and bring it to your medical doctor or nurse practitioner to sign.
How Long Does It Take to Book an Appointment?
Your referral will be processed as soon as it is faxed to 647-351-6648 (our Requisition Processing and Calling Centre). Referrals are triaged based on the cardiac status and the information provided by your referring physician.
Once we receive the referral, our office will contact you to schedule an appointment and provide the date and time. Patients with urgent concerns will be prioritized and contacted the same day during business hours. Please ensure that your referring physician or nurse practitioner has your most recent and updated contact information.
Note: Our clinics are closed on statutory holidays.
